In order to complete the following documents,

Please download Adobe Reader (a free PDF viewer) at this site.
Open each form by right clicking on it, selecting “open with” -> Adobe Reader.
If you have trouble viewing Adobe PDF, open the PDF in desktop app.
Fill each form by typing all responses except for signatures.
Print each document as it is completed.

​Adobe Reader sometimes will not allow you to save your work, so it is helpful to have a printer on-hand to print each filled document. Upon completion, documents are to be mailed to the address given in the NSO 2024 Welcome Packet.
